NEW YORK (AP) — New York City’s Administration for Children’s Services says its involvement in the case of a 4-year-old girl who was severely underweight when she died was woefully inadequate.

The agency released its preliminary report into the case of Marchella Pierce on Tuesday.

ACS Commissioner John Mattingly testified before the City Council. He said the documentation in the case was insufficient and home visits were inadequate.

Prosecutors said Marchella was beaten, and police say she may have been tied to a bed. She was born with lung problems.

A Brooklyn grand jury has indicted her mother on second-degree assault charges. Carlotta Brett-Pierce’s lawyer did not immediately return a call seeking comment.

    Just look at those faces and you can tell that they are criminals. What is goverment doing assigning these peolpe to be care givers. THis is only in NY, where goverment gives a lot of $$$$ to people without investigating who they really are. May she rest in peace…

    @ jd: Sometimes you can get a clean conviction on a lesser charge and put someone away, while you will lose them, for lack of sufficient evidence if you attempt a greater charge. Prosecutors balance these things out when they make charges. It’s hard for us to watch, but the point is to make the case so the jury can convict and the person serves time.

    this is unfortunate and sad what happened to that little girl. some people are ment to NOT have kids. the child service system is a huge failure. at times i think that people who run that place are not qualified. this isn’t the first time something like this happened and someone turned a blind eye.

    the baby has died and the mother gets only assault charges??
